A nurse-led lifestyle program designed to help you reset your lifestyle habits, improve your health, feel stronger, more energized and more in control of your health while reducing your risk of developing lifestyle related chronic diseases like heart disease, stroke, type 2 diabetes and even some cancers. Welcome to Your Habit Reset Project —a personalized, evidence-informed program where we focus on simple, sustainable lifestyle changes that support your long-term health. Expect to walk away from this program with ideas, insights, lifestyle habit changes, and sometimes new ways of showing up in the world.

What to Expect: Your Habit Reset Project is a 3–6 month program designed to fit into your busy life—you set the pace. The program includes 12 guided learning modules and workbooks, personalized action plans, and 10 one-to-one coaching sessions delivered via telehealth. Delivering the right system, support, and accountability, you’ll move from where you are today to where you want to be with your health and wellness goals. Alongside your Nurse Coach, you’ll be supported every step of the way as you work through three structured phases of transformation.

Phase 1: Creating Awareness
We begin by building awareness through a series of guided learning modules designed to help you define your health goals and clarify your why—your core source of motivation. In Module 2, we explore practical strategies to boost your energy – where you’ll learn simple, effective strategies to boost your energy so you can have the capacity to make lasting lifestyle changes. providing the foundation needed to support sustainable lifestyle changes. Module 3 focuses on transforming your relationship with stress – one of the most overlooked barriers to good health.

Why prioritize stress in Phase 1? Research shows that chronic stress—something most of us experience regularly—is a significant driver of systemic inflammation. In turn, systemic inflammation is now recognized as a major, preventable contributor to chronic diseases, including heart attack, stoke, and type 2 diabetes.

Phase 2: Take Charge
This phase focuses on the key lifestyle habits proven to reduce the risk of—and in some cases help prevent—the development of chronic disease such as heart disease and stroke, diabetes, neurodegenerative diseases like Alzheimer’s, high blood pressure, and certain cancers.

Through a series of engaging learning modules, you’ll take a deeper dive into anti-inflammatory living, including topics such as What the Heck Should I Eat?, Hormones and Weight Loss, Food & Mood, Move More, Sleep Well and What is Healthy Blood Pressure. Each module is designed to equip you with practical, evidence-informed strategies that you can confidently apply to your daily life.

Phase 3: Change for Life
In this final phase, the focus shifts to sustaining meaningful, long-term change. Research in Lifestyle Medicine highlights the vital role of connection, purpose, and self-care in achieving lasting health and well-being. This phase brings together the whole person—body, mind, and spirit—to support a balanced and fulfilling life.

Through a series of reflective and practical modules, you will explore topics such as Building Connections: The Importance of Social Wellness, Developing a Self-Care Plan, and Planning Your Next Chapter. Each module is designed to help you integrate what you’ve learned and confidently move forward with habits that support your health for life.

The Short Course: ‘Let’s Cut to the Chase’

This program is a shorter, 4-module / 4-week version of Your Habit Reset Project. It is designed for individuals who are ready to make meaningful lifestyle changes but feel stuck and want additional support to move forward. If you know what you want to achieve but have struggled to get started or stay consistent, this program provides the structure, guidance, and accountability to help you take action.

Together, we will clarify your goals, ensure they are realistic and achievable, and develop a clear, personalized plan to move you forward with confidence. With focused support and accountability, you’ll have what you need to finally bridge the gap between where you are and where you want to be. Includes:

  • Four one-to-one coaching sessions ideally one week apart over 4 weeks.
  • Three foundational learning modules to set you up on the right path for success: determining your health goal and what your why is; finding your energy to make change, and dealing with stress in a healthy way.
